Update: March 24, 2025
We have updated a number of forms on March 23 to reflect the new Additional Deposit Option (ADO) and comply with Payments Canada requirements:
Starting April 24, 2025, old versions of these forms will no longer be accepted. For all requests involving the Additional Deposit Option, the new versions will be mandatory.
Please use the online versions of our forms to avoid using outdated copies.
What’s changing
Paying for the insurance / Payments and premium instructions sections
The pre-authorized debit agreement portion have been removed from these sections. If the person wishes to pay by pre-authorized debit, they must complete the Pre-Authorized Debit Agreement (PAD) – 09312E.
To make sure you don't forget, the forms clearly indicate that the PAD form must be completed if pre-authorized debit is selected.
Pre-authorized debit payments
The Pre-Authorized Debit Agreement (PAD) – 09312E form is the only form that can be used to authorize pre-authorized debit payments from an account.
The PAD form contains more required information and gives us clearer instructions for pre-authorized debit payments. It contains sections with the following information:
- The amount and frequency of the pre-authorized debit payment: This section must be signed by the payor
- Instructions for applying the payment amount: This section must be signed by the policyowner.
It is also possible to indicate an amount for a one-time payment.
Impact on your work
New insurance applications – Paper and PDF forms
If you use paper or PDF application forms and you’re offering multiple coverages, a separate copy of the Pre-Authorized Debit Agreement (PAD) – 09312E form must be completed
for each illustration you produce, in order for payments to be made by pre-authorized debit.
Changes to insurance policies with pre-authorized debit
Fill out the required form for the desired changes. If the person paying for the insurance wants to pay by pre-authorized debit, they must complete the Pre-Authorized Debit Agreement (PAD) – 09312E form.
To ensure insurance applications and requests for changes are processed as efficiently as possible, the PAD form must sent together with the other relevant forms.
